Outsourced RT Creates Increased Profitability Under PDPM
Recently, an article appeared in the Skilled Nursing News that specifies how outsourced RT creates increased profitability under PDPM (the Patient-Driven Payment Model). The article quotes Edwin Frost of Aeris Consulting, regarding the potential for increased profit for nursing homes; you can read it here.
As the article notes, PDPM matches reimbursement for nursing homes based on resident acuity. Frost points out that in the wake of long term acute care hospitals closing, skilled nursing facilities have been taking on sicker patients with more complex needs. With PDPM, credit will be given to nursing homes for caring for these sicker patients.
While in the past caring for more complex illnesses had been a cost of doing business, there will now be greatly increased reimbursement potential for skilled nursing facilities. A facility needs to demonstrate at least 15 minutes of face time between respiratory therapists and patients each day, and the facility must also document a patient’s condition during a specified look back period. With timely respiratory therapy through outsourcing, the article also points out, nursing homes can also prevent costly hospital trips for patients.
All of these changes make outsourced RT creates increased profitability under PDPM for skilled nursing facilities. Frost stated for the article that consulting services typically cost about one-third of the total reimbursement that providers accrue, making for as much as a 70% return on investment. In addition, outsourcing is an alternative to in-house training for staff nurses that can only see a few patients per hour, and who could simply leave in an area where there is high turnover.

Ed Frost Speaks About The Potential of PDPM
The piece is focused on the increased potential for profitable care in the era of the Patient-Driven Payment Model (PDPM). The author of the article, industry expert Alex Spanko, spoke with both Ed Frost of Aeris Consulting and Melissa Sabo of Gravity Healthcare Consulting, regarding PDPM and what it means for skilled nursing facilities.
As detailed in the article, more complex respiratory therapy for sicker patients is becoming a key revenue source for facilities. As Frost points out, “Skilled nursing facilities over the past several years, and this has really amped up in the past five years, are being forced to take sicker and sicker and sicker patients — especially with many areas of the country losing their long-term acute care hospitals.”
Frost added that “There’s very little access to higher levels of care, intermediary levels of care, and they really have been forced to take sicker and sicker folks.” He noted that with PDPM, nursing homes will now receive credit for admitting sicker patients that require higher levels of care.
Frost also emphasized that it’s not necessarily a fee-for-service play…with a timely intervention from a respiratory therapist, a nursing home operator can help to prevent return trips to the hospital for patients. Cardio pulmonary problems are a very common source of hospital readmissions.
The piece took note of the potential for growth in skilled nursing facilities as a result of decreased readmissions. A skilled respiratory therapy facility can save on the high costs of hospital readmissions, which has a positive impact on a facility’s reputation. As Frost pointed out, “Readmission rates start to go down, their marketability starts to improve. Amongst patients and family members, when they’re in the facility, confidence levels increase.”
